What you need to know about esophageal cancer

Esophageal cancer grows unnoticed for a long time. Difficulty swallowing usually appears when the tumor already narrows the passage, so many patients learn the diagnosis at an intermediate or late stage. Risk factors differ: for squamous cell carcinoma they are smoking and alcohol, for adenocarcinoma – long-term heartburn, obesity and Barrett’s esophagus (a condition in which the lining of the esophagus changes under the effect of stomach acid). Monitoring and treatment of this condition are handled by gastroenterology clinics.

The type of tumor matters for choosing the approach: squamous cell carcinoma and adenocarcinoma respond differently to radiation and drugs, so their treatment regimens differ.

How esophageal cancer is diagnosed in Israel

Surgery. For a locally advanced tumor, an esophagectomy is performed – removal of part of the esophagus with the lymph nodes. The operation is done as open surgery or minimally invasively, including with robotic assistance. Chemotherapy or chemoradiation is most often given beforehand.

Chemoradiation. The combination of radiation and chemotherapy is used before surgery, and in squamous cell carcinoma in some cases as the only treatment.

Drug therapy. In advanced disease, chemotherapy, targeted drugs for HER2-positive tumors and immunotherapy are used. Immunotherapy is also prescribed after surgery if tumor cells remain in the removed tissue.

Supportive treatment. A stent or radiation helps restore the passage of food when the tumor cannot be removed.
