Stages and forms of bladder cancer

About three quarters of bladder tumors have not grown into the muscle layer when they are first found. They are called non-muscle-invasive. Such tumors rarely threaten life, but they often come back, so they require regular check-ups.

Muscle-invasive cancer grows deeper, can spread to the lymph nodes and other organs and requires more serious treatment. Carcinoma in situ is a separate form: a flat lesion of the inner lining of the bladder that is hard to see during a routine examination but tends to progress. With recurrences or doubts about the histology, German clinics often review the samples in their own laboratory.

Tumors located in the bladder neck, the part of the bladder that joins the urethra, are treated according to the same principles, and bladder neck cancer treatment in Germany is planned by a tumor board based on the stage and depth of invasion. Small cell bladder cancer is a rare, fast-growing form. It is treated in the same way as other small cell tumors, with chemotherapy at the center of the plan, and stage 4 small cell bladder cancer treatment in Germany relies mainly on drug therapy.

Diagnosis of bladder cancer in Germany

Diagnosis is based on cystoscopy, an examination of the bladder from the inside with a thin instrument. German urology clinics widely use fluorescence cystoscopy: before the examination, a substance is introduced into the bladder that makes tumor areas more visible under blue light. This helps find small lesions and carcinoma in situ.

In Germany bladder cancer is treated only after the stage has been determined precisely. To assess the spread, CT urography and CT of the chest and abdomen are performed, and pelvic MRI or PET-CT if necessary.

Bladder cancer treatment methods

Non-muscle-invasive cancer. The tumor is removed during TURBT. Then drugs are instilled into the bladder: a chemotherapy drug right after surgery and BCG (a weakened tuberculosis vaccine that activates the immune system against the tumor) in courses over months or years. If the tumor comes back despite BCG, new intravesical drugs or bladder removal are discussed.

Muscle-invasive cancer. In stage 3 bladder cancer treatment Germany follows the standard approach: cisplatin-based chemotherapy before surgery and radical cystectomy, removal of the bladder with the lymph nodes. In Germany, this operation is often performed with a robot. Urine drainage is restored through a urostomy on the abdominal skin or with a neobladder, an artificial bladder made from the bowel. After surgery. If tumor cells remain in the removed bladder or lymph nodes, immunotherapy may be prescribed to reduce the risk of the disease coming back.

In stage 4, the tumor has spread to distant lymph nodes or other organs. Treatment in this case is drug-based, and it has changed noticeably in recent years. Bladder cancer immunotherapy uses drugs that remove the brake from immune cells and allow them to attack the tumor. Today it is often combined with an antibody-drug conjugate (a drug that delivers chemotherapy directly into tumor cells). Platinum-based chemotherapy followed by maintenance immunotherapy is also used, as are targeted drugs if changes in the FGFR3 gene are found in the tumor.

Immunotherapy for bladder cancer in Germany is prescribed after assessing kidney function, general condition and previous treatment, because these determine which regimen suits the patient.
